We are a specialist provider of supported living services in the Northwest of England, offering 24 hours support and move-on packages to individuals with a learning disability, Autism and a mental health diagnosis through a person centred approach. We can provide bespoke packages of various degrees, ranging from scheduled visits and monitoring within the clients own home to full 24 hour supervision with assessment and intervention from specialist multidisciplinary team members including Psychologist, OT, Speech and Language Therapist, Recovery and Employment Specialist etc. As an organisation, we have special expertise in understanding and rationalising the care of individual from long stay, complex and hospital inpatients, particularly those with Intellectual Disability or Autism, who need to be moved on as part of NHS England's Transforming Care Agenda. Our Ethos is that each service user will have their needs individually assessed and a support plan will be agreed with them. We put great emphasis towards the prevention of placement breakdown and hospitalisation aiming for a progressive route towards independence. Our staff are skilled to assist service users with their housing related issues and available to support them with their daily living skills to live a more independent lives in the community. Also skilled to manage any risks that they may present to self and others. We have a core complement of experienced clinical staff including a Consultant Psychiatrist and Learning Disability nurses who will be responsible for the supervision of the clients care, depending on the needs and risks of the individual, this may include a weekly review by the MDT. Our services are registered with the Care quality commission and have a robust internal governance programme to ensure we provide the best quality of support to all the individuals
