The Maritime Confidential Human factors Incident Reporting Programme

International Maritime CHIRP receives reports from mariners of all nationalities on ships of any flag about hazardous incidents or safety issues which the mariner may be unwilling to take up with the ship manager or the authorities. CHIRP reviews each report, discusses with the mariner a proposed course of action, and then follows up the report accordingly. Selected reports may, with the mariner's permission, be included in CHIRP Maritime Feedback, a journal that is published on the website www. chirp. co. uk so that safety lessons can be promulgated more widely in the maritime community.
