"It's not about dementia. It's about people. It's not about services. It's about living life. It's not only about talking. It's about listening, thinking and doing together". We believe Universal Personalised Care has the potential to revolutionise every aspect of living with dementia. NHS Long Term Plan identifies Universal Personalised Care (UPC) as a principle driver for improving dementia care. For real change to happen we need to move policy into practicalities in ways that recognise the unique experiences of people living with dementia. We need to adopt different ways of working together as a united team, reaching across the boundaries of social care, health, and the voluntary and community sectors. The DCAN is a new open and collaborative space where we bring together people living with dementia, carers, researchers, innovators; strategic leaders and implementation sites. It's about encouraging a positive, proactive approach to transforming dementia through the components of Universal Personalised Care. We are creating the spaces and platforms to learn from and support each other to build the action orientated relationships required to turn the best of current thinking into effective change. Our intention is not to reinvent or replicate, but to build upon what already exists by choreographing and co-ordinating relevant work across complex systems and multiple organisations.
