About Disability Initiative
Disability Initiative started life in 1975 as Camberley Association for the Disabled. It was formed by an Occupational Therapist as a community-based service for people with a physical disability.
Disability Initiative Description
DI provides a high standard of care and social rehabilitation that recognises the personal dignity, choices and freedom of the individual, whilst working towards maximising independence and responsibility.
A client can be referred by a Care Manager or other third party such as a GP
or even refer themselves directly to DI. Following an initial assessment by DI’s
Occupational Therapist, an individual care plan (ICP) is drawn up that ensures
the necessary day-to-day care, agreed goals and choice of courses, therapies,
activities, independent living or return to work skills that suit each person.
This ICP is regularly reviewed by their allocated keyworker and adapted as
a client progresses. All of our services are supported by qualified staff to allow
DI to measure outcomes and ‘distance travelled’ as a client works towards their
agreed goal.
