TERMS OF REFERENCE OF THE HODNET PATIENT PARTICIPATION GROUP

Title of the Group

The Group shall be called THE PATIENT PARTICIPATION GROUP (PPG) of the HODNET
MEDICAL CENTRE and is affiliated to the National Association for Patient Participation
Membership No 1713.

The Objectives of the Group

 To improve communication between the Practice, patients and the community it serves, and in particular to give feedback to the Practice about patients’ concerns and expectations.
 Provide an avenue for patients’ input in the way facilities and services are planned, and executed and to influence those services.
 Support the Practice in good health promotions, preventative medicine and health literacy
 To be a “friend” to the Practice by providing feedback on the community’s needs, concerns and interests, and challenging the Practice constructively whenever necessary.
 To promote co-operation between the Practice and Patients to the benefit of both.

Membership of the Group

The Group shall normally consist of no more than eight (8) patients of the Practice, and this should aim to
reflect a cross section of the patient body without discrimination, together with the representatives of the Hodnet Practice.

Activities of the Group

 The Group will be kept informed of the Practice policies relating to the Primary Care Trust
to which it belongs. It may express opinions on these policies on behalf of the patients.
 The Group will consult with the Practice on service development and provision and assist in the assessment of community medical needs.
 The Group will contribute to, and be kept informed of, Practice decisions.
 The Group will encourage and support activities within the Practice to promote preventive medicine and healthy lifestyle choices.
 The Group will assist in informing Patients of the work of the Practice and activities of the Group through the Hodnet and Practice websites.
 The Group will seek to ensure that Patient information and advice are readily available and clearly presented.
 The Group will represent patients at the Practice in seeking to influence local provision of health and social care.

Meetings of the Group

The Group will endeavour to meet four times a year with the Practice and will hold an informal preparatory meeting in each quarter.

Organisation of the Group

 The Group’s activities will be organised by a Committee of invited members, a Chair, and Secretary.
 Administrative assistance will be provided by staff at the Practice.
