"DEGENERATIVE DISC DISEASE". It sounds awful doesn't it! It's actually just the medical terminology for a normal thing. This was my response to a recent blog article by a chap considering spinal fusion 😨😱
"Hey there. I am sorry to hear your story. I work as a spinal specialist physical therapist. I can't answer your question quickly but I can try to begin by dispelling some myths.
... Degenerative disc disease in modern science is regarded as a normal finding on everyone's imaging as we age. In multiple research trials where people with NO lower back pain were MRI scanned, DDD was found in many of the people from ages 20 upwards. It got more with age and is now regarded as a normal sign of aging just like getting grey hair and wrinkly skin.
Pain is always produced by our brain. 100% of the time. Please check out a great website www.retrainpain.org for more information on up to date pain science.
Back surgery like the ones you mentioned does not work. In the UK they recommend NO fusion surgery for this type of problem. If you want to read from a US spinal surgeon turned advocate check out www.backincontrol.com
Pain killers can be helpful in the short term but have been proven ineffective in the long term.
Please don't think anyone including me is saying that your pain 'is all in your head'. However our brain is the thing that produces pain.
We have no pain receptors in our body. So in the same way that our ears don't hear (they just get vibrations) and our eyes don't see (they just receive light information), our nociceptors (sensory receptors in our bones, muscles, tendons discs) can only relay sensory information into our brain. The brain if it deems that there is danger produces pain.
I work with people who have persistent pain daily. It is almost always NOT to do with the state of their tissues (discs etc).
There has normally been a lot of past unsuccessful 'treatment' options given to them, a lot of bad science and old school advice. There is usually a lot of fear, stress, frustration and anger around the pain persisting.
So what does work?
Learning about pain to reduce fear
Getting gradually more active over time within your limits using exercise.
Using relaxation and breathing to unwind your nervous system
Trying mindfulness to calm the mind.
Having a good guide
You would be amazed at the success stories that patients have. They are normally skeptical at first but by the end normally say 'i wish i had done this years ago.'
You CAN do it. Find a good guide and make a start. It will be a bumpy road but you can do it. Good luck"

Was recommended by a friend after I'd had recurring shoulder problems following dislocation and surgery. Ross dedicated a lot of time determining the best course of rehabilitation and then designed a really useful preventative program for me to do in the gym. After only 5 sessions and following his recommendations I was back in better shape than before surgery, and also fixed up other niggles I had with my back too. Great physios, can really recommend anyone with sports injuries to go see them.

This place was a true blessing on Christmas Eve. Yesterday my dad went go karting and hurt his back, over night it seriously deteriorated to a point where he could hardly walk. This morning (Xmas eve) everywhere was closed including Hogan and Mitchel.



However, after speaking with Simon (at the physio therapy center) he came from his home and saw my dad at the last minute. Leaving his family the day before Christmas to do so.



This center is not just a physio therapy center but clearly a place that cares as well!

Ross sorted out my shoulder injury successfully.

My shoulder had troubled me for some time and suddenly became much worse.

Having diagnosed shoulder impingement, and specifically which muscle groups were affected, Ross gave me a set of exercises.

He subsequently fine tuned the exercises over a couple more appointments to target the troublesome area.

This did the trick and the condition cleared as the exercises took affect.

Quality service! I needed an appointment as soon as possible so I could get back to my fitness programme and they delivered with great results. Simon was a great help being able to pinpoint the cause of my problem and showed me how to relieve it. He created a specific rehabilitation programme tailored to me so that I would be fit again in no time. Follow up checks via email were great to ensure I was on the right path to recovery. I’ve consulted Simon with four different ailments over the last 6 years, and each time he’s treated them hugely successfully.



They’ve ranged from sciatica, caused by bad posture at work, to an ITB problem brought on by riding 1000 miles on a bike in 10 days. Each time Simon’s technique has been the same – he’s asked me lots of questions, and asked me to undertake apparently simple, but controlled movements. At first you might wonder where it’s all leading; the answer, as it turns out, is to a fantastically accurate diagnosis of the problem, the likely cause, and to a range of movements or stretches that if practised diligently will sort the problem out.



Where Simon differs from other physios I’ve experienced in the past is that he takes nothing at face value; he takes the time to understand your symptoms, and really goes back to first principles to understand what could be causing them. I recommend him strongly, and without hesitation.

It is no exaggeration to say that for the last 20 years I have been suffering from back pain; it had become so entrenched in my lifestyle that I had come to accept it as an inevitable by-product of being tall. Simon initially suggested pilates group lessons which were wonderful and went a great way towards alleviating the pain and introducing flexibility. Following that I have had a series of individual pilates sessions with Simon addressing my back and shoulder pain: this has had an unprecedented effect! Simon has effectively trained me in a in a series of pilates based exercises that have greatly enhanced my core strength and flexibility. I have not enjoyed so much suppleness for years and although I still do still have stiffness and some pain it is hugely better. I feel motivated to participate in my own treatment and practise the pilates exercises at home. These sessions form an important and enjoyable part of my week and I would thoroughly recommend this course of action to anyone suffering from physical pain.

I've been doing Pilates here for about 6 months and love it! I've suffered from back pains for a while and Pilates has really helped. My weekly session is great for releasing tension in my back and I now have a bank of exercises that I can do at home too! Staff are really friendly and helpful and always willing to share advice.

I started attending pilates at Hogan and Mitchell around 5 months ago. I started the classes as I have struggled with lower back and hip pain for many years now, and needed a solution to ease the pain without having to resort to surgery on my hip.

Since starting the class my pain has eased (in both my hip and back) so much that I don't even think about it any more, and I haven't had to visit my sports therapist for treatment in months. My muscles feel like they are constantly improving, both by building my core strength as well as stretching my 'over worked muscles'.

The class I attend is taught by Vicky; it is fun, varied and I come home every week feeling fitter and reinvigorated. Vicky is exceptionally good at giving each individual attention when needed to correct posture and to help us improve each week.

I continue to attend the class with enthusiasm; it isn't a chore (as I thought pilates would be), it is something I really look forward to each week!

I signed up for one of the Pilates courses at Hogan & Mitchell Physiotherapy last month, in an attempt to help build strength in my lower back from an old injury. They were recommended by a friend who had a similar problem. I've only been going for a month but I can already feel a positive difference. The classes run by Stacey are great - she provides an excellent balance of both group and individual attention, and is clearly incredibly knowledgeable and proficient at what she does (good fun too). Following excellent advice from Simon for pain in my lower back, my partner and I started Pilates with Vicki over a year ago now. My partner had previously had a back operation which left him with balance problems so it was important to have a physiotherapist who clearly knows what she is doing and keeps an eye on everyone during the exercises. We have this in Vicki and much more, especially a lovely manner and a good sense of humour! Sounds corny but he quite literally transformed my life. His knowledge of muscular skeletal issues are second to none. I would very much recommend him.

I had persistent pain in my ankle and Achilles, after twisting my ankle running off-road down-hill. I decided to try Hogan and Mitchell when the pain did not resolve itself and I was unable to run without pain. Simon took a refreshing approach as he did not take my account of the issue at face-value, and worked with me to get to the root cause of the problem.

He demonstrated his diagnosis to me through getting me to do balancing and other exercises which gave a bench-mark for improvement. It was a very different approach from many physios, who rely more on ultrasound and other interventions.

I felt that Simon's approach was to work with me to develop new habits and fix the problem for the long term.

I was very impressed with his follow-up between sessions and his openness in explaining the approach he was taking.

I am now running 3-4 times a week again and have completed a couple of races. More importantly I am gaining confidence in not twisting my ankle again due to improved strength and balance and I have a set of exercises that I can build into a weekly routine.
