Locala Community Inreach Team Description

In Kirklees the Admissions Avoidance Service is in its infancy. The service went live at the beginning of April this year. Our objective is to identify patients who do not require admission into secondary care (hospital) and utilize clinical expertise and community experience to facilitate appropriate community interventions through discharge to an enhanced crisis intervention community team. The A& E teams are based at Dewsbury District Hospital and Huddersfield Royal Infirmary and we coordinate care between a dedicated Intermediate Care Team, Rapid Response and a Hospital Avoidance Team to ensure that this integrated approach enhances the service a patient receives from both a health and social perspective. There has been an immediate and noticeable positive impact since the team has been in post and our motivation includes providing a high standard of care to the public as well as a reduction in cost associated with emergency admissions into secondary care including length of stay. The Admissions Avoidance Teams are instrumental in improving communication between community health and social care teams, GP practices, secondary care and the Yorkshire Ambulance Service and our presence in the A& E department has also improved the link between primary and secondary care.
