Our care at home service allows you to remain independent in the comfort of your own home, without compromising on the quality of your care. Precision care provide transitonal services, from hospital to home, leaving care to independant living. Our services provide support for attending work placements, further education and daily living skills. We have a range of supported housing accomodation for the following client groups.
Older people with support needs.
Adults with complex learning difficulties which may exist alongside challenging behavioural and physical impairments.
Adults 16 + Transitional care
Young Adults (aged 16 to 25) with support needs
Young adults with Autism or Asperger’s Syndrome
Single people with alcohol and /or drug dependency or may have a history of engagement with the criminal justice system
People with mental health problems
Brain aquired injury
