In the spine, manipulative therapy most directly affects the facet joints, which are synovial joints that allow our spine to move in all three planes of motion (sagittal, frontal & transverse). Like other synovial joints, the facet joints ...contain synovial fluid and are surrounded by a fibrous joint capsule composed of connective tissue. - The facet joints are often thought to be a source of pain and often respond quite well to various forms of joint mobilization. What you should know, however, is that the popping sound you hear when having your spine manipulated/adjusted is nothing more than a pressure change in the facet joints, just like popping the joints of your hands. It has NOTHING to do with correcting the alignment of the spine. Any practitioner who claims otherwise and tries to convince you that 'tune-up' treatments are necessary to maintain proper spinal alignment is simply not reading the research or more interested in their bank account than your health. - At the end of the day, research has demonstrated time and time again that manual therapy can be helpful, but the effects are largely short-lived. This systematic review of the literature was to determine the current level of evidence available for the use of exercise therapy in the treatment of full-thickness treatment of RTC tears.
There were 2010 shoulders in 1913 subjects with an average age of 64.2 years, 54% males, 73% of tears were >1 cm and ...37% were classified as massive, >58% of tears were >1 year and 73% were atraumatic...wow, chronic RTC tears! ✅78% improved in pain ✅81% improved in ROM ✅85% improved in strength ✅84% improved in functional outcomes
Dissatisfied outcomes occurred in 15% of patients, who then transitioned to surgery. They ultimately gave a GRADE B recommendation (moderate strength) to support the use of exercise therapy in the management of full thickness RTC tears. There seems to be a trend to try a course of PT instead of rushing into surgery for many injuries that have historically been only treated with surgical intervention. What do you think? While there is a lot that goes into this question, two things I usually mention are that disc herniations are common in much of the asymptomatic population and that they can heal with time and conservative treatment.... . The following is a case report from the New England Journal of Medicine that documents the resolution of a lumbar herniation without surgery. . 📕A 29-year-old woman presented to the spine clinic with new-onset pain in her right leg, accompanied by paresthesia. MRI of the lumbar spine revealed a lumbar disc herniation resulting in substantial spinal stenosis and nerve-root compression (left image). . 📕She elected conservative treatment with physical therapy and an epidural injection of glucocorticoids. A second MRI obtained at follow-up 5 months after presentation showed resolution of the herniation (right image). Her clinical symptoms resolved, and she was discharged from the clinic, with follow-up recommended as needed. . 📕Data from clinical trials suggest that patients who have herniated lumbar discs have similar long-term outcomes whether they undergo surgery or elect conservative management.
