CHALLENGING HYPOSPADIAS SURGERY
It is nice to get feedback from patients. I think I remember this one- had been operated by the best centers in Europe. But then hypospadias surgery is like that- humbles the best of us at times.

Surgeon talking neurology --------------------------------- Last year a student joined me to do her Master’s thesis. We followed up patients treated 10 years ago for NEC necrotising entero-colitis at our centre, with the aim of assessing their neuro development.
Overall mortality was 31%. 61% had neurological impairment at 10-22 years age. Very few studies have ever published such a long term followup. We saw cerebral palsy, blindness, learning diff, autism....
... As a surgeon, I am a GI plumber- I join bits of tubing. Often at age 5, I have discharged the patient. But as is clear here, someone somewhere is picking up the pieces. I called it the hidden burden we bequeath to the community !

A very important paper written by me which confirmed my long standing view that the Internal sphincter sparing approach was not a good operation to treat Ano-rectal malformations.

... Abstract There are several contentious issues in the management of inguinal hernias in premature neonates. They include the timing of surgery, the management of incarceration, the role of routine contralateral groin exploration and the type of anaesthesia. In this article we re-emphasize the importance of delaying surgery until the baby is ready for discharge from the neonatal unit. Regular examinations and manual reduction of these hernias should be performed in order to detect incarcerations early. CONCLUSION: With this approach, it may be possible to minimize the morbidity resulting from incarceration, and at the same time avoid the respiratory and testicular complications resulting from an early surgical/anaesthetic intervention.
