A massive well done to all of my friends who completed the #paras10 in #colchester yesterday. I’m proud of all their commitment and the way they pushed their bodies to the absolute limit.
British Paratroopers are the most elite airborne unit in the world. The 10miler is an event born out of the necessity to parachute in behind enemy lines and then cover vast distances to engage the enemy in battle.

Runners this will interest you if you use a foam roller...
Most runners will have heard of the ITB or suffered from a condition called ITB syndrome. The old advice of massaging or foam rolling the ITB is dished out on forums and passed down from more experienced to novices in running clubs all the time.
But this study demonstrates that you'd be better off foam rolling the glute musculature that connect and converge into the ITB fascia. Participants in this study had almost 1...5% improvement in hip range of motion than those that just foam rolled the ITB.
Next time you choose to use a foam roller put some method in the madness.

Pain is not just about the bone, joints and muscles (even though it's hugely important). There's an increasing body of evidence that suggests contextual factors have a role in musculoskeletal pain. Rossettini et al (2018) identified five psycho-neurobiological mechanisms of contextual factors. These can effect brain networks, neurochemistry and the outcomes of treatment.
1. The clinician characteristics.
The type of clinician, their appearance, whether they're physically fi...t or not, how they communicate and motivate the patient physically through touch and verbally.
2. The patient characteristics.
Age, gender, weight, physical condition, pain level, function level, the patients memories, emotions, expectations, optimism, previous experiences, & pyschological characterisitics can have an effect on pain.
3. The patient / therapist relationship.
This includes the rapport built between the therapist & patient. The way in which the therapist communicates to the patient. The way the patient responds, their body language, tone of voice and apprehension.
4. The health care setting.
This is the environment in which the treatment / encounter takes place.
5. Treatment.
This speaks for itself. Whether it's manual therapy, exercise or a congnitive behavioural therapy approach. The way it's administered, the pressure, touch, the patients past experiences of the modality.
When I see a patient I take into all of the above so that I can build a strong rapport with my patient. I believe that if we can build a strong alliance then I can better educate and motivate them to put themselves into the driving seat to manage their pain more effectively.
