The Clinical Commissioning Group is seeking the views of children and young people who have been diagnosed with or are awaiting assessment for a neurodevelopmental disorder (such as Autism, ADHD, tic disorder). It may also be filled in on behalf of a child or young person by a parent or carer. This survey aims to understand the experiences of those who have been diagnosed, or who are currently waiting to be assessed. The survey aims to learn what works well at the minute and how we would make things better. The deadline for completions is June 27th 18.
