- to confirm an appointment I will take your name and contact number. This is essential should I need to reschedule your appointment due to unforeseen circumstances. ... - During the initial consultation You will complete a client record card which will contain your name, address, contact number, email, date of birth, health/medical information and any contraindications. I require this information for insurance purposes and in case of emergency. - I will ask you to sign a data card at your next appointment. This card is to go along with your medical data and is now required by law if I am to store it. - This information is recorded and kept for legal and insurance purposes. It is kept In a locked filling cabinet in my home salon which is also secured. - All information is kept for a period of 5 years from the initial consultation and will be disposed of by shredding. - I do not use your personal information for anything other then to contact you regarding your appointments. - I will not pass your information to any third party. - I will not use your personal information for any marketing purposes. - You may ask that I delete any messages received regarding your appointments. - Should a security breach occur you will be notified immediately. - Should you wish to withdraw consent for the above at any time please note that I will no longer be able to treat you.
