Our improvement team worked with Sue Ryder's care-at-home service in Arbroath to develop and test a new way of documenting the condition of clients at each home visit, to pick up on any deterioration and alert seniors so they can enable faster access to assessment and treatment.
Home carers tested the tool, making changes at each stage of development and clients and their families got involved in the testing too, giving valuable feedback.
