Empowering patients to self manage their long term health conditions.

Berkshire Healthcare and Bracknell & Ascot CCG are working in partnership to engage patients and the public to deliver an approach that ensures patients feel more empowered and involved in their own care as well as having patients engaged in the work we do.

We want to bring about fundamental change and build a community based on our patients’ needs and wishes. We want relationships between health professionals and their patients to be collaborative. We also want to recognise the benefits of integrating engagement across partner organisations that are often competing for the same individuals or duplicating work.

The project has been successful in creating a small pool of trained HealthMakers who are able to facilitate self-management training for long term health conditions and /or offer guidance and advice to various work streams including other local NHS organisations, Bracknell Forest Council, Healthwatch and other voluntary organisations. They've helped improve the quality of life for over 50 people to date and hope to achieve a pool of more than 200 trained HealthMaker Facilitators in the long run.
